A patient who is at a risk of having Subacute Bacterial endocarditis had a Third molar surgery at your clinic. Patient is known allergic to Penicillin and derivatives. On the day of surgery you gave Clindamycin 600 mg P.O 1 hour before procedure and since surgery was extensive so you continued Clindamycin 300 mg P.O every 6 hours for another 5 days.
Patient comes back for suture removal now you will

Give Azithromycin 500 mg PO 1 hour prior to surgery

Give Doxycycline 200 mg PO 1 hour prior to Procedure

Give Clindamycin 600 mg IV 30 minutes prior to the procedure

Give 600 mg Clindamycin PO 1 hour prior to the procedure
